Healthy Roster

Upper St. Clair School District utilizes Allegheny Health Network to provide athletic training services for our athletics. Part of that contract is the use of a software Healthy Roster. This is an electronic medical records system to help manage pre-participation paperwork, track injuries and treatments and communicate with parents/guardians.


Healthy roster is available via mobile application and desktop website.

Creating a Healthy Roster account:


Email the following information to any of the three athletic trainers:

The athletic training staff will notify you upon account creation. There will be an email from Healthy Roster to create your log in.


Pre-participation paperwork:

All paperwork must be reviewed and approved by the athletic training staff prior to participation in any tryouts, practices or games. The deadline for submitting documentation is two weeks prior to the official start of the season. (i.e., the start of track season is March 2nd, everything must be submitted by February 16th) Once the Healthy Roster account is set-up, all documentation is found in the documents tab. Paperwork must be completed on the official PIAA CIPPE forms.

Multi-Sport Athletes:

Multi-sport athletes must submit a Section 7: Sport recertification form prior to the start of the second or third sport seasons. This form gives a chance to update the athletic training staff on contact information and injury updates.


To fill out the section 7 form: Log into Healthy Roster

  1. Documents Tab
  2. “Add Document” or the “+” button on the mobile app
  3. Click on document type
  4. Scroll down to Section 7 and select the appropriate form (Winter or Spring)
  5. Fill out and sign the form
